possible exposure to HIV symptoms and window period

JKK
Dear Doc,

First of all thank you very much for helping everyone in the forums. I'm a married man without children. My situation is a little different from what I have searched and read. I have Genital Herpes since years. I had an outbreak possibly a week before a protected sex with a (Brazilian) sex worker in the Netherlands on 11th of March. The sores were not completely healed. She did the condom on and gave oral (with her teeth going too rough on the surface of the condom) for a while and then inserted. After being finished, she would pull the condom a little with a tissue paper and clean my penis with the same tissue paper. 6 or 7 hours later I had unprotected sex with my wife and on the 3oth of March she had soar throat which lasted for a week.I had very slight sweating a couple of nights. She tested negative (HIV and all other STIs, except the Genital herpes which she did not test) on the 11th of April. Around 22 or 23 April she had headache which goes still on. She would normally always have headache for a day or two after her every period this time she had her last day of her period around 20 or 21 April. But this time her headache has lasted way longer. Now will you please advice me over what percentage of risks have I had. Do I or my wife need to test at 8th week (two months)?. Thank you ver much once again.

I had sex with a prostitute with alot of deep kissing, she did perform oral on me very breifly without a condom. Now I am having alot of discomfort in my abdominal area, ie: pain, nausea ,constipation and generally feel like ****. Should I get tested or am I freaking out. Could it last weeks? Need to know I am married and wouldn't want to hurt my family. Please help.

I had a sexual intercourse with a pros and used condom. That was the very first time and last time in my life that I had sex with someone other than my wife. It was in the month of August 2006. Since then I have been feeling very bad for my action.
The pros took away the condom after it is done with a cloth and cleaned mine with the same cloth. That night was a hell for me and cried the whole night in my hotel room for doing such a worst thing in my life.
I need to know are there any possibilities of being HIV+. I did not have a test until now. My wife was carrying and the doctor had to send her for all tests including HIV. We have tested her in the month of April, 2007. The result was negative. My fear is that I might be in the window period and possible of getting HIV+. Can you please help me in this regard. I cannot express it to anyone. No one not even my best friends knew about this and I am struggling a lot in my heart. I had cold and cough in the month of November. Last 10 days I had suffered with cold only and last night that is 1 June I had night sweat(offcourse it is very hot here in my place). Are these symptoms for me being a HIV. I cannot even go to a diagnostic center as most of the people recognize me in my place. My situation is such.

Thank you very much for your reply. I will not have follow up question at all but this one:

Do you think that the symptoms at the end of the second week and the middle of the sixth week mean nothing? AND Do you think even an unhealed Genital Herpes soar cannot be a route of entrance for HIV from the fulids of outer side of the condom which came in contact with that tissue paper. Thank you very much once again. I willl have no more follow up questions I promise.

You had condom protected sex; how the condom was put on or taken off, and the vigor of oral sex, don't change the zero risk of the event.  Your symptoms do not suggest HIV.  From a risk assessment perspective, testing doesn't make much sense; I never recommend testing on account of such low risk exposures.  But the simple fact that you are worried enough to come to this forum to ask the question indicates that you need to have a negative test result for reassurance.  Eight weeks is plenty of time. You will sleep better after you are tested and found to be HIV negative.  Your wife does not need testing, however; your negative result will prove you weren't infected in March and therefore could not have transmitted HIV to your wife.

Good luck--  HHH, MD
