risks from hand jobs and spitting on penis for lubrication?

Doctor,

About a month ago, I visited a massage parlor and received a hand job.  The girl spit on my penis for lubrication.  Is there a risk of getting HIV or another STD from the hand job or from her spitting on my penis?  This week I had a fever and chills and sore throat, followed by a rash on my face, chest, back, hands, feet and buttocks area.  My one year old son had hand foot and mouth disease so my symptoms may be due to something acquired from him.  But I fear the worse.  Is there any risk to acquiring HIV from the hand job or the spitting on my penis?  Should I get tested?  I visited my general practitioner and a dermatologist during the last few days.  They believe the symptoms are due to some viral infection but were not overly concerned.  I did not tell them about my visit to the massage parlor.  Your guidance would be appreciated.

Hand-genital contact has never been known to transmit HIV, even when genital secretions are used as lubrication. And saliva carries even less risk.  Saliva kills the virus, which is why HIV is rarely if ever transmitted by oral sex.  In addition, the chance any particular sexually active woman has HIV is very low, probably under one chance in a thousand.  This also applies to massage workers who may double as commercial sex workers.

Given your son's illness and the professional opinions of two doctors that your symptoms weren't a cause for worry, it seems likely you indeed you were sharing your son's garden variety viral infection.  I see no need for HIV testing or for any further concern about your massage parlor experience a month earlier.

Just as oral sex carries much lower risk for STD transmission than vaginal or anal intercourse, saliva uncommonly transmits STDs.  While the risk may be higher than for HIV, it remains miniscule.  In 35 years in a busy STD clinic, I never saw a patient who appeared to have acquired any STD by hand-genital contact, regardless of use of either saliva or genital fluids as lubrication.
