sex with sex worker

Hi Dr Hunter
I had unprotected oral both ways with a female sex worker in asia,followed by intercourse with a condom.as i was fairly drunk,my concerns are regarding the correct putting on and removal of the condom as these are not clear in my memory though the used condom was by my bed in the morning and was not damaged.could you give me your opinion on the chances of me having caught hiv from this scenario?have you had many cases of hiv from one off one night stands with prostitutes?and lastly,what would your opinion be on a negative result with a duo test at 21days?
i appreciate your time and apologise for what must seem like the same tedious repetitve questions from idiots like me.

This will be my last question.
I got Hiv duo test at day 21 and it was negative!
tonight will be my first day without sleeping pills in 3 weeks.
i will go back in 7 days time for another Duo.
are you aware of 21 day duo tests ever changing up until 28 days and would you be VERY confident in my position.
If you have time i would appreciate your opinion ,if not,i will post my result anyway.thanks again

Dr. Handsfield and I share the forum.  You got me.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

Let's work through your questions one by one.

Let's start by acknowledging that it is unlikely that your partner had HIV. Most commercial sex workers do not and, even in the unlikely situation that she did, most exposures do not lead to infection.  The risk of getting HIV from genital-genital sex with an infected partner is only 1 in 1000.  The quoted figure for HIV risk, if one has oral sex (giving or receiving) with an infected partner is less than 1 in 10,000 and, in my estimation that is too high. Some experts state there is no risk at all from oral sex.  Neither of us on this site have ever seen or reading the medical literature of a convincing instance in which HIV was passed by oral sex.  

My guess is that the condom was used properly.  Most CSWs are very careful about correct condom use in order to protect themselves.   Even if it was not, your risk of infection is very, very low from the exposures you describe.  

Indeed, I have seen people who have acquired HIV form a single exposure. These people can only be characterized as being very unlucky.

A negative DUO test at 21 days is VERY strong evidence that you did not get HIV, particularly when considered along with the other factors mentioned above.  We do not know precisely what proportion of infections would be detected by the DUO assay at 21 days but certainly most of them.  At 4 weeks the results of a DUO test can be considered definitive.
