The meds seem to not be working

My Girlfriend is HIV pos with an aids diagnosis. For the last year her tests have been at undetectable levels. However she has recently been complaining of fatigue and is worried that her present treatment isn't working anymore. I don't know exactly what drugs she is taking. She is a MTF and is on estrogen therapy so I know that her doctor is using drugs that have less interference with that part of her overall medical care. In the past when she went off of her meds she deteriorated very quickly and I am scared that if the meds stop working she will again.
