Test or no need to?

Hello. I wanted to explain my actions first before I ask some questions I had an encounter on Feb 7 and I used a condom but when we changed positions I slipped out of it and it was hanging in her vagina. I took it off and I blew into it to check for breakage and there wasn't. I put it back on and I continued and climaxed inside her.  She took a oraquick yesterday and she came out negative but she tells me that she had 3 sex partners that two were protected except her ex.  But she says he's negative cuz he gets tested for his blood pressure and some other stuff.     I also had unprotected oral three times with this girl.    Now in this scenario would you even consider this a risk? And also how come some pole say oral is a risk but there's also those who say it isn't.    I also did her in December but I wasn't worried about that cuz I had a swab test with her to last week and I came negative which means that if I had something it would've shown up by now.    My encounter in Feburary is my only concern and was wondering if there was any risks in my actions:
Reusing the condom, me slipping out of it (and I remember that my penis did not touch her vagina), and oral sex risks, and if i took the oraquick swab too early or if i never needed to take the test.

I really need help shaking this feeling out if my mind I can't take these headaches and chest pains anymore I feel like I just wanna die.
