56 days ECLIA negative, no penetration, help needed

Dear doctor,

I am a male from China, 32.
More than 2 months ago, I went to a so-called bath center in a small city of China.

there, the lady massaged me with her body, and then her licked my whole body including my anus, smoothely, which I don't know whether she condomed her tongue when she licked my anus. at last, she gave me an oral sex with condom on me, as I ejaculated, she stopped oral and took off the condom, handjobbed a few seconds to the last. But during this, i touched her clitoris, smoothely, there was little secretions. and immediately after the finger touch, I checked my finger, no visible wounds.

NO real (neither vaginal nor anal)sex after all.

I began to worry on 1 December, 2011, which is the international hiv day. I tested for several times:

the latest is one weeks ago, 56 days negative.The test was conducted in the biggest hospital, ECLIA, antigen and antibody combined test.

before that, I did the same test in the same hospital on the 35 days and 43 days. Both negative.

on the 31 days, I did another 2 tests in another 2 hospitals, one is ELISA, the other is a quick one. Both are negative.

but about one week ago,  the right upper ending gum, which has no tooth ( may be the wisdom tooth's position, i am 32.).
today the pain is serious and when i swallow, i can feel the pain, more, when i press the neck closely under my ear with force, it also hurts, but I can't touch lumps or something like. I don't get fever.

Welcome to the forum.  I'll try to help.

This initial response is based on nothing but the title of your question, before reading anything else.  HIV test results overrule everything else in determining whether or not someone has HIV, if done sufficiently long after the last exposure.  A negative result is more reliable than symptoms, exposure hsitory, or anything else.  The title of your question suggests that you are concerned you might have HIV despite a negative HIV ELISA antibody test 56 days (8 weeks) after exposure.  That proves with nearly 100% reliability that you did not catch HIV.  It doesn't matter whether or not your partner was infected, and it doesn't matter what symtoms (symptoms) you might have.  The test resuls rules!

Now I have read the rest.  Guess what?  The test is reliable.  And in any case, you had a very low risk exposure, since HIV is rarely if ever transmitted by oral sex, and certainly not by massage, hand-genital contact, anal-oral contact, etc, etc.  Further, it is very unlikely your partner had HIV.  The large majority of commercial sex workers (99+%) are not infected.  Finally, negative rapid HIV antibody tests are just as reliable as the lab-based tests like ELISA.

So you can stop worrying, and stop having HIV tests.  This was a zero risk sexual exposure and the test results prove you didnt' catch it.

your words are powerful. I am very glad to communicate with a world top expert. I still have some questions waiting your kindly reply.

1. but my test is named ECLIA, in China they (hospital and CDC) call it "Chemical lighting methods", one the report printed "antigen and antibody combined method", does this matter the results?

2.and most professionals in china say 3 months windows, do you see or hear somebody turn positive after 8 weeks negative test?

3. you say I can stop testing, does that mean I can have unprotected sex with my wife? (she is faithful to me, and i feel very guilty to her,sorry).

4. I also did syphilis test on the 43rd day and 56th day,all are nagative. is that conclusive?(sorry this question is out of range of this title)

1) All antibody tests are equally good, in virtually the entire world.  I'm not sure exactly what ECLIA stands for, but from your desciption I assume CL is chemoluminscent, and IA probabl is immunoassay -- whereas ELISA is enzyme-linked immunosorbent assay.  When you spell them out, they don't look much different, right?  And they aren't.

3) Yes, you can safely have sex with your wife.  This conclusion is not based primarily on your test result, but on the fact that you did not have an exposure that could have resulted in HIV.  My advice would be the same if you never were tested.  (In fact, I would have advised you NOT to be tested at all, if you had asked before you did it.)

4) You also could not have caught syphilis; those tests also were a waste of money.  But in any case, the results are reliable; 6 weeks is the standard recommendation for syphilis testing.

Please try to stop worrying about this.

Thanks, dear doctor.

I am trying to convince myself, but I am still worrying something else, for example, the comdom was possibly broken during the oral (yes you said unprotected oral is rarely a risk), and maybe there were invisible wounds on my finger. I know you might think I should go to psychologist, but I love my wife and baby so much, and I feel guilty to them, and I am afraid of transmitting HIV to them, that is why I want to be 100% sure. Sorry.
