The blood on the needle is on the suface area where it is exposed to the elements whereas, blood in the INSIDE of the bore of the needle is protected from the elements.
This is why a simple superficial needlestick is not a risk, where iv drug users sharing syringes IS a risk. They inject the infected blood directly into their bloodstream.
