Dear Dr H.,

I had circumcision operation done when I was a teenager. I am now in my early 30s. The surgery left a few permanent suture holes that produce “blackheads” that require attention from time to time. There are maybe five or six permament holes all the way around just below the head of the penis. I wanted to find out if these holes in my skin were vulnerable to HIV or other STDs.

The reason I am concerned is I had oral sex with a condom on at a massage parlour. The sex worker there took the condom off to finish with a handjob. I got paranoid and lost my erection completely. I told her to put the condom back on, but by this time, since my penis was totally flaccid, she had trouble getting the loose condom back onto my now small penis. It took her a couple of moments, but finally she wrapped it around most of the head and part of the shaft and began sucking to let my penis grow back and expand into the condom. I was concerned since part of the condom touched tip and head of my penis as she was trying to refit.

1) Did I practice safe sex?  I feel like i messed up and should've just let her finish with a handjob without the condom.

2) Do you think I have any need to test for HIV at all?

3) Are my suture holes in my skin vulnerable to HIV or other STDs

Thanks dr. Any insight on this will definitely help me.

1) Receiving oral sex is always safe, probably because saliva inactivates HIV--so even if your massage partner had HIV, there was no risk.  And she probably wasn't infected anyway.  

2) From a risk assessment perspective, you definitely do not need testing.  But if you need to know the negative result to help reassure you, it's fine with me.

3) I don't see any reason why the post-circumcision lesions you describe would alter the risk of HIV, particularly in this circumstance.

You totally practiced safe sex.  Hiv is not transmitted from hand to genital, i.e. handjobs or fingering. The risk for oral sex is too little to worry about.  Plus you had a condom on(for the most part.)   You were EXTRA safe if you ask me!

I went ahead and tested for all std's at 11 days after the exposure, and everything came back ok.

So it's been over 7 weeks now since the fellatio incident late last year. I've taken an oraquick at 6 weeks and came back negative, as expected, and it did help ease my mind.

Between the 4th and 5th week, I developed an oral blister/canker type of sore on the inner part of my bottom lip area, but inside my mouth and it's lasted for a couple of weeks, and i still have it. Is there any reason for me to be concerned that it's syphilis related?

Any input would be great, thanks.

i picked up a tranny on the streets and paid her for a handjob. upon ejaculating i had this fear of any possibilities of contracting HIV. it was quite a messy handjob and i am currently fearing of the circumstances. there were no saliva contact but when the handjob was performed, it was without a condom. i sort of caressed his penis but only once or twice. is it possible if; i more often than not bite my fingernails that would leave a small wound at the tip of my fingers contract HIV while carressing his penis? much appreciated thanks.
