Condom broke, concerned.

I went to a local Los Angeles Thai massage parlor. After a quick massage, I engaged in sexual intercourse and about 5 minutes in the condom broke, I didn't notice for about 5 seconds and I pulled out to discover what had happend. I didn't ejaculate inside her but I was wondering if even that short amount exposed leaves me at risk to any STD's, HIV, or pregnancy.

She was 45 years old and told me she hadn't had intercourse in 10 years. She said she was clean. But she didn't speak english very well.

Do I have anything to be concerned about?

Read innumerable threads on this forum about the low risk of single episodes of vaginal sex, even if the woman has HIV, and the relatively low rate of HIV in most commercial sex workers in the US and Europe.  There are no clear data to answer your question, but there is no reason to believe you are at high risk.  So from a risk assessment perspective, you don't need testing for HIV.  But if you will sleep better having a negative test, you can do that about 6 weeks after the exposure.

Beyond HIV, would that limited amount of exposure give me reason for anymore concern of other STDs? Is pregnancy a concern from the pre *** even with her age?

Pre-ejaculate comes from the cowper's glands, and a few small studies have shown it doesn't contain any sperm. Or rather, it shouldn't, unless you haven't urinated since you last ejaculated. Regardless, it's sweet of you to care, but I can't see where pregnancy would be your responsibility. Occupational hazards and all that.

And on that note, I'm not buying this "haven't had intercourse in ten years." It's a pretty, uh, sketchy claim. She's a *sex worker*. Not to mention going without for that long wouldn't just be coincidence, or one of those "things"; it would have to be purposeful and would have to have SOME meaning. It just seems awfully unlikely that she would just suddenly break a 10-year long fast with a one-time client... just a thought.
