Condom usage

After looking at this forum and the doctors forum, I've noticed some differences between the stats and views on common use. If someone could shed some light on some things for me, I would be grateful.

1) Do you need a test/see your doctorif your condom does not break? Is it truly safe sex?
2) Condom are said to be 90% effective. Does the other %age pertain to failure rate?
