Fellatio + mycosis + Chlamydia / high risk of getting HIV ?

Dear Doctors,

First of all, I would like to congratulate you concerning the quality of your web site. It is very useful and well presented.

Despite the many posts I have read, I am still unable to evaluate the risk I took. Here are my concern and questions:

I am a 28 years old gay male from France and I would appreciate to have your opinion on my last sexual relationship.

Here s the story (please excuse my English):

About two months ago, (end of March 2008) I went to Miami USA and had sex with this latino guy. As a top I penetrated him (with a condom and lubricant of course) and he gave me a blow job. The thing is that at that time I noticed that my glans was a little redder than it usually is. This “redness” became more and more acute the following days and my glans was seeping. One week later, once back to France and quite worried to be honest, I had an appointment with my generalist and I was diagnosed with having a “benign mycosis’’ and he gave me a treatment (cream of econazole). The mycosis completely disappeared in four weeks.

When I asked my doctor how this could happen to me? (Because I’ve never had this before in my all life!) He only said that this is very common thing and that I did not need to worry about it.

This being said, last February I’ve also been diagnosed with whooping cough and I have been cured with macrolides. When I was in Miami, I was precisely taking the macrolides’ medication to cure. My generalist told me that macrolides’ treatment can sometimes lead to those side effects but I can’t help to doubt him since nothing like that was indicated on the insert.

Nevertheless, I decided to get tested for all sexual diseases at the hospital and it happened that all results went negative excepted concerning Chlamydia which, according to the infectious diseases specialist, was a “little positive’’. In other words, it means that, when I had this sexual relationship in Miami I was Chlamydia positive! Anyway the infectious diseases specialist gave me doxycyline to take for 14 days. He examined my penis and he also confirmed that yes it was a “benign mycosis’’. When I asked him if this could be a symptom of Chlamydia he only said that yes it could be as well it could not …

So here are my first two questions:

1) What is your opinion: According to you, this so-called “benign mycosis’’ was due to the macrolides’ treatment I took in order to cure the whooping cough I had? or
2) It was more likely a kind of a consequence of the Chlamydia?

I allow myself to highlight the fact that the diagnosis of whooping cough had also been confirmed with a serology on last February. Diagnosis of ARS had to be abandoned when all tests – from research for Agp 24, PCR, to ELISA tests all went negative – I had only been vaccinated for whooping cough when I was a child in 1981 and that’s all. Doctors explained me that now they have discovered that vaccinations for whooping cough are not efficient for a life time as they now see many young adults with this pathology.

This is being said, my third question is:

3) To which point this “benign mycosis’’, as well as the fact to have been Chlamydia positive during my last sexual relationship in Miami, increases the probability to get HIV? Once again I indicate that when this happened my glans was a little redder, not seeping yet, and that I did not know that I was Chlamydia positive otherwise I would not go to this sexual intercourse. Only the blow job I got from that guy was unprotected. Anal sex was protected.

I got tested at four then at eight weeks (56 days) after this sexual relationship. Each time the method used was divided into two different techniques as follows:

• HIV technique n°1 (HIV ½ gO – Abbott – Axsym) ………..……….. NEGATIVE
• HIV technique n°2 (HIV Ag/Ac* – Combo – Abbott – Axsym)……..NEGATIVE

* Ag = French abbreviation for antigen / Ac = French abbreviation for antibodies

I Spent hours on the internet doing researches. Acute information is hard to find when official recommendations say it is necessary to wait 12 weeks (84 days) to be a 100% sure of a negative result and when, on the other hand, many reliable documents say that 6 weeks is now enough with the new testing techniques…

… On your website I found out that the time of doing the test has to be compared with the risk taken. I agree with that but the thing is that I am not simply able to qualify the risk I took. Normally (meaning no mycosis and no Chlamydia) I would say that the risk is very low but regarding the information I gave you, I am now completely lost! This leads me to my last questions:

4) Do you know the techniques used on my last tests (Abbott / Axsym)? Are they reliable?
5) Do you think a 8 weeks / 56 days test compared to the risk I took is enough to completely peace my mind or should I wait for the 12 weeks (84 days) test ? If so, can you please try to quantify the probability that those negative results finally turn positive? (1 on 100? 1 on 1000? 1 on 10 000?) I am sorry but those last three months (the former tests I had with the whooping cough and later the mycosis…) have been so stressful that I now trying to get the more information possible from specialists.

Doctor, I am so scared you cannot imagine. I know I acted like a fool but I did not know that I had Chlamydia and mycosis. I need your honest opinion. Of course I am hoping for good news but I am especially hoping for an honest and reliable opinion. Many thanks in advance to have read this long message.

Those 8 week antibody/antigen test results are almost impossible to change positive at a later time. By all means go back at week 12 if you want to to ease your mind. Sounds like you were just sick prior to the sexual encounter anyway.

Besides getting a blowjob isnt a risk anyway.
