HIV Risk?

Hi everyone,

I would appreciate your advice for my situation. had unprotected oral and protected anal with a male stranger in the early morning of 12th April (2 am). performed oral sex on him for about five minutes, and he pushed my head down to deep throat him a few times which made me gag and feel a bit raw at the back of throat. then had protected sex with condom (I saw him put it on) and lube for about five minutes. then he took off condom and used his finger to force my lips open to cum in my mouth. i couldn't stop him from doing that as it happened too fast. i quickly spit out the semen a few seconds later.

I developed a sore throat the next day in the late evening (13th April), and it has continued to worsen into a blocked/runny nose and dry cough now. I am having a bad headache and lethargy too. I am worried that I have caught an STD, or maybe more than one as it was a place for anonymous sex and i did not ask his HIV status

My concerns are because:

1. i have chapped and slightly bleeding lips because of the cold harsh winter/spring. i also brushed my teeth about 30 mins before the encounter.
2. i did not ask his HIV status, and being in an anonymous backroom for sex, he could have anything (he certainly seemed familiar with the surroundings and people there)
3. i have not had a sore throat or been sick for more than a year, and this sickness came so suddenly. Could it be a fast-appearing symptom of gonorrhea, chlamydia or syphilis?

4. most importantly, i fear HIV transmission due to the reason of my chapped and slightly bleeding lips, and also the brushing of teeth 30 mins before and 15 mins after the encounter.

I would appreciate it a lot if i could get advice on this, and to calm my fears too especially about the HIV scare. I live alone and I am very very scared now. Thank you for understanding and for your help...

To Teak:

I'm not sure that you speak for the moderators or regulators of MedHelp. But what I do know is that I ask truthfully from the point of a concerned individual. Isn't that what MedHelp is for? If there is no room for discussion and it's just a matter of asking someone to get lost after you've given him a piece of your mind, then this isn't quite the forum that it's supposed to be. It doesn't make sense of you to state your stand and then get upset when someone offers a different viewpoint from another certified medical doctor. Also, I've witnessed your bullying tactics towards others who have similar concerns, and from a fellow human being, I don't think that's nice of you. At all.

To Nursegirl:

Thank you for your reply and information. Your response was certainly much more measured, thoughtful and is deeply appreciated. It is people like you that help assure the deep fears patients have, not just with information, but with kindness and love.

Thank you. :)

We cannot help that there is conflicting info out there, and do not debate the info found elsewhere.  As I say all the time, there are definitely varying opinions on the level of risk for oral sex, although even the sources that state it is a risk will state very low, or negligible, which means it's not worth worrying about.  I explain IN detail in the link Vance provided why oral sex isn't a risk.

As for protected anal sex being a risk, that's just ridiculous.  If a condom is on and intact, there is no opportunity for transmission.  You asked for our take on your situation, and we gave it to you.  You have to decide what to believe.

Kindly move along.

By the way, I hope you meant that doctors on this site say it is NOT a risk?

Hey Vance,

No offence meant really, that would be the last thing I would want to do. All I want is to seek a peace of mind. The website is askdrtan.com, and this Dr. Tan specialises in answering online questions regarding HIV as well. Maybe you could take a look and understand what I mean.

Nothing more for us to tell you. No clue who this "dr" is but Dr's on this site who are specialists say it is a risk.

You can't keep seaching the net, finding something and then come back here. You either believe us or you don't

Hi Vance, nursegirl and everyone,

I got very worried after a doctor forum (askdr tan.com) told me that I am at risk for HIV and STDs even with protected anal sex. It gives me one more thing to fear now after my worry about the unprotected oral sex has been calmed.

Could you pls advise me on your thoughts on this pls? And what are the risks of transmission with a condom used properly? I am sure that it was there from the beginning to the end. Thank a lot in advance

Thank you all for your advice.

Can I ask another question then? I have not had a sore throat, flu or been sick for more than a year, and this sickness (sore throat that also developed into blocked nose with mucus and a cough) came so suddenly. About a day and a half after the encounter. Could it be a fast-appearing symptom of gonorrhea, chlamydia or syphilis?

Hi Vance,

Yes, and no offence meant as well. I am just puzzled because there are some doctors who say there are risks through bleeding lips and open wounds, and some who don't.

Please understand that I am not trying to contradict you guys, but I just wanted to be clear on this and feel more assured. The contradictory statements online have puzzled me a lot

Hi Teak,

Thank you for this information. I acknowledge the advice given by doctors like Dr. Hook, Dr. Garcia and Dr. Hansfield, but this seems to be contrary to what other doctors and forums on the internet are saying, that there is a risk of transmission through bleeding lips or open wounds?

Could you or others kindly clarify this pls? I would appreciate your help ver much. Sorry if this is taking up your time, but it means a lot to me :)

There is no debate (among experts) about the HIV risks associated with oral sex. The risk is so low that almost nobody who cares for HIV infected patients has ever had a patient believed to have been infected that way. Among experts, it's a semantic issue about using terms like "no risk" and "very low risk". There is no difference between my or Dr. Hook's use of "low risk" and other experts' "no risk".
DR. HANSFIELD

"And oral sex is basically safe sex -- completely safe with respect to HIV and although not zero risk for other STDs, the chance of infection is far lower than for unprotected vaginal or anal sex. Please educate yourself about the real risks. If you stick with oral sex and condom-protected vaginal or anal sex, you have no HIV worries and very little worry about other STDs. " DR HANSFIELD

"I am sure you can find lots of people who believe that HIV is transmitted by oral sex, but you will not find scientific data to support this unrealistic concern..." DR HOOK

"HIV is not spread by touching, masturbation, oral sex or condom protected sex."- DR. HOOK

in the public HIV Prevention forum of MedHelp, TEAK and the other moderators maintain that oral sex in all forms is a zero risk activity. Would you agree with this assessment?
I TOTALLY AGREE / DR GARCIA
"HIV is not spread by masturbation, through oral sex, through kissing or other casual contact." Dr. Hook
"The observation on thousands and thousands of observations is that HIV is not spread by oral sex (of any sort)." DR HOOK
"I would not say your risk ,if he had HIV is "slim to none"- that's too high.  I would say they are effectively zero.  How much of his ejaculate or other genital  secretions you may have swallowed makes no difference.  EWH "
"As far as HIV is concerned, there is no known risk of getting HIV from performing oral sex on an infected partner, even if that person's genital secretions get into your eyes or if you swallow." Dr.Hook
HIV is not spread by oral sex, giving or receiving, even if sores, gum disease or blood is present
DR HOOK
The fact is that there are no cases in which HIV has been proven to be transmitted by oral sex, including fellatio..  EWH

Hi nursegirl,

Thank you so much for your reply to me. I am a little puzzled when you say that HIV cannot be transmitted through small cuts and bleeding gums even with semen, because I have read doctors' comments online and HIV help websites that HIV can be transmitted orally through this kind of situation. Could you please advise me further on this? I don't mean to ask for the sake of asking, but I am genuinely puzzled by the different stances taken. I would appreciate it a lot..thank you..

No, you cannot get HIV through small cuts and bleeding gums, even with the presence of semen or preseminal fluid.  You would only ever be at risk for HIV with unprotected vaginal or anal sex.  NOTHING else (sexually) is a risk.

To add on, my concern is because the precum and semen probably did come into contact with the small cuts caused by brushing and the wounds from the chapped lips. Thank you once again

Hi Vance,

Thank you for replying so promptly. I appreciate it. I'm a bit confused regarding your comment about the lack of risk as I have been reading that brushing of teeth (that in turn produces small cuts in the mouth and gums) and chapped lips that lead to small open wounds will lead to a risk of HIV transmission. Could you or someone kindly advise me further on this please?

Thanks and I appreciate your reply very much

I don't know how someone could force your mouth open and ejaculate in it so quick that you had to do it. Anyway, you did not have a risk. Saliva, air and other things will deactivate the virus. Brushing teeth and chapped lips would not give a route for infection.
