HIV from a special case of fellatio

Hi,

I had posted this question on your STD's forum by mistake so here goes again. A few days ago i was in bangkok and i visited one of this soapy massage parlors. This was my first time and my interaction with one of the girls there involved penetrative vaginal sex with a condom on and after which she went on to give me a fellatio with the condom on. She then spat out the fluids in her mouth and then went on to continue the fellatio after she removed the condom. We then went on to have penetrative sex with a new condom. Now these parlors claim to be registered and have their girls tested every 3 months and use condoms with every customer but i am guessing the possibility of her having HIV is very high. With that in mind what is the realistic risk of me getting HIV, Particularly when she was exposed to her own vaginal fluids in her mouth(on the outer side of the condom) spat it out and gave me a fellatio without a condom? Please answer this query as i am petty worried.

Your risk is very low.  For starters, I suspect that someone has checked and the CSWs in these massage parlors do not have HIV. Statistically, worldwide, most CSWs do not have HIV.  

Whether she had HIV or not however, your risk of HIV is close to zero. The quoted figure for HIV risk, if one has oral sex with an infected partner is less than 1 in 10,000 and, in my estimation that is too high. Some experts state there is no risk at all from oral sex.  Neither of us on this site have ever seen or reading the medical literature of a convincing instance in which HIV was passed by oral sex and we find that many, many of our clients have been exposed to HIV infected partners through oral sex.  In your own situation, not only is the risk tiny but a condom was used for most of the exposures making the risk lower still.  That she may have had some small amount of her own vaginal fluids in her mouth when she performed unprotected oral sex on you does not change my assessment.

Bottom line- close to zero risk.  So low that there really is not a medical reason for you to be tested for HIV.  If it would give you peace of mind to have a negative test result, please wait until at least 4 weeks following your exposure.  At that time you can have confidence in the test result as verification of my assessment.  Hope this helps. Thanks a lot doc. The primary concern was for the vaginal fluids in her mouth as i feared she may have not efficiently spat it out. From what i read on sites vaginal fluid on the penis soft tissue is a high risk situation of HIV transmission so i was a little freaked out. The order mentioned by them was i think
1)Blood
2) Vaginal fluid)
3) pre-*** fluid
etc.
