Hello Doctor.First at all sorry for my bad english but i'm writing from Italy.So i'm a man 36 y.o. in good health.I never any dangeorus sexual encounters in all my life.Just last year i went to Japan for about 4 months, so i'm writing to you about what happened.
I met a japanese escort 2 times, so we engaged of course vaginal sex with condom but oral sex (her to me) unprotected all the 2 times.Also i met another girl in a club and we had sex many times always covered with condom but this time i also did oral sex to vagina and anal to her (maybe the right word is rimming).She also did oral sex to me (unprotected)with ejaculation.Now i'm in italy and i find a nice girl.I love her and we decided to do sex without condom in the nearly future.Doctor do you think i need some  test about HIV?My doctor said don't worry because i used safe sex so i don't need any kind od test.I did HIV test 2 years ago and was negative, after that no more sex before going to japan.So just 2 partners after HIV test.I wrote to you because i know you are an expert so you can reassure my mind.Thanks in advance.

Welcome to the HIV Prevention Forum.  I hope that I can provide you with information that will help you.  To provide you with a summary of my thinking to start with, I think your risk for HIV is very, very low.  A more detailed response is found below.

To summarize your note, you have condom protected sex with several women in Japan as well as receiving and giving unprotected oral sex and rimming of one of your partners.  The risk from these activities is low.  HIV rates is Japan and relatively low and while escorts are high risk partners, despite this the large majority of them do not have HIV.  Thus it is unlikely that any of the partners you mention had HIV.  You cannot get HIV from un-infected partners.

But, what if they had HIV??  The quoted figure for HIV risk, if one has oral sex with (giving or receiving) an infected partner is less than 1 in 10,000 and, in my estimation that is too high. Some experts state there is no risk at all from oral sex.  Neither of us on this site have ever seen or reading the medical literature of a convincing instance in which HIV was passed by oral sex.  the risk from rimming is about the same (very, very low).

So, what should you do .  There is no great medical reason for you to have an HIV test.  At the same time, if your get an HIV test now, it will put your mind at ease and relieve you of any lingering doubts you have about the exposures you mention.  for that reason, I suggest you go on and get an HIV test.  I am confident that the results will be negative and at that time you will be able to move forward with your relationship with no doubts whatsoever.
