HIV test accuracy: Broken condom and stressed

by Jayson386, Oct 24, 2009 11:05PM
Hi

I recently had sex with a working girl in Australia and noticed that the condom had broken when I withdrew. I hadn't ejaculate nor did I have any cuts/sores on my penis. I was really worried and she informed me that she was clean and that the requirement for testing is every 3 months and a swab test every three weeks. I got in contact with the local sexual health clinic who advised me that it was a "low risk" and that HIV amongst registered working girls are extremely low. The person at the clinic informed me that the wait had to be 6 weeks before testing and the average is 25 days.  The question: I had a test at the 25 day mark which was a HIVag/HIV1/2 which came back negative and that was great news but I am still concerned that there may a chance of risk. Is this type of test accurate for this time frame(25days) and sholud I do another test or consider this as conclusive?. The doctor at the clinic said that based on my risk assessment that he wouldn't usually recommend a test but for me peace of mind it would be best. I would really appreciate some advice

Welcome to the HIV forum.

No worries, if you don't mind I use a common Australian phrase.  The chance a particular commercial sex worker in Australia has HIV is very low; and your partner reassured you (and people rarely lie when asked directly about HIV status).  And even if she were infected, the odds of transmission are very low for any single episode of unprotected vaginal sex.

Australia's sexual health centres are, collectively, the best STD/HIV clinic system in the world, and the odds are that your providers there know a lot more about all these things than we do on this forum.  Specifically your statement "The doctor at the clinic said that based on my risk assessment that he wouldn't usually recommend a test but for me peace of mind it would be best."  That is precisely my advice, which I have given hundreds of time on this forum.  I could not have said it better myself.

If by chance your care is at the Sydney or Melbourne SHC, say hello for me.  I am close friends with some of the senior physicians at both clinics.  (Do you recognize the location of my photo?)

Cheers.  All is well.   HHH, MD

Thanks for the advice.

Could you please confirm the accuracy of my test at the period I had my test and what are the propabilities for me to catch HIV.  For example would you say that the test is 90%, 95%,100% etc. accurate and is this test the best to have for early detection or should I have another one. I would really appreciate your feedback.
Thank you

Chance your partner had HIV, no more than 1 in 1000. If she was infected, chance of transmission 1 in 2000.  So even before you were tested, the likelihood you caught HIV was 0.001 x 0.0005 = 0.0000005, or 1 in 2 million.  By 25 days, probably 90% of newly infected people would have positive tests.  So at this point in time, the likelihood you caught HIV is 1 in 20 million.  For practical purposes, that is zero.  For anxiety relief, you might want to have another test at 6+ weeks after the event, for the additional reassurance from another negative result.  That test also will be negative.

Follow the advice of your SHC provider(s).  They know what they are talking about.  Since you are in the hands of real experts -- who likely know more than I do about these things -- I won't have anything more to say.

G'day.

Thanks for your advice as it really made me feel much better as well as made me move on without any concern. I have had a constant dry throat for the past three weeks and wanted to know if it had any relevance to HIV or not. Would love some advice.

No relevance et al.  Test results and risk assessment ALWAYS outweigh symptoms, and those show you virtually could not have HIV.  Therefore, it doesn't matter what symptoms you might have; they are not due to HIV.  And "dry throat" doesn't suggest HIV anyway.

That's all for this thread.  Any further questions can be directed to your SHC providers.

Hi Dr Hunter.

I wanted to follow up on my last result. First of all Thank you for the advice, I did have another Ag/Ab test and it did came back negative as you said. It did help with the anxiety. Can I assume that this is over or do you recommend a follow up test after a set period ?

Thank you
