Need advice on risk of transmission...

Hello,  sorry I'm ignorant to this stuff as this is new to me...   About a month ago I met up with a sexworker.  The entire encounter was covered.  She started with condom BJ and removed the condom and used another condom for intercourse.  After finishing, she removed the condom and cleaned me with a warm towel (I can't remember how the condom removal was done...).  She then masterbated herself to orgasm and licked her fingers.  We talked for about 10-20 min and the we started again.  She rubbed some lubrication or (handsanitizer?) on my penis began giving me a handjob.  She then placed a condom on me and we began intercourse again.  When I finished I went to the bathroom to take off the condom.    Here are my concerns:
1.  When she was giving the the handjob and her vaginal fluids came in touch with my penis then the condom went on, can this transmit HIV if her vaginal fluid was mixed inside the condom?    

2.  I'm concerned that when i removed the condom, I touched her vaginal fluids on my penis.  The concern that I have is that later I realized that the tip of my circumsized penis was rubbed raw possibly from the friction.  Can this be a concern for transmission from her vaginal fluids on outside of  the condom that come in touch with a penis head that may have been rubbed raw?

3.  I had a small cut on my lip ( I tend to bite, peel skin off my lip) off 2 hours before meeting her.  We did not kiss but I did suck her breasts, kiss her legs and neck area.  Can I get HIV from this or from any fluid that may have been on her body to my lip?  I didn't go down on her...

4. I don't think the condom broke or slipped off at any point but... what are the chances/risks if it did?

I recently got tested and the results were back negative but I've heard there is a "window" period.  I've recently had a fever, stress, anxiety, night sweats, fatigue.  I'm scared to death that I have contracted this and may passed it on to my wife of 8 years...

Please help answer the questions. I am just freaking out???

Thanks for your quick replies.  What if there was a cut on my penis that I didn't know about?  Can vaginal fluid infect me?
