Possible HIV exposure from Oral Sex during Period

Dear Doctors:

As I type this message, my fingers are loosing strength and actually trembling at what your response may be.

Yesterday was a beautiful day. I had a productive day at work. Came back home happy and I did something I never usually do. I opened a bottle of hard liquor and started drinking away on an empty stomach. Had lots to drink.

I called up a sex worker. She came to my place; a middle aged caucasian woman who wasn't as presentable as she claimed in her post. My hormones were gushing like I can't explain and her lack of presentation, so to speak, did not make me change my mind. I'm a 27 year old male.

We did not have sexual intercourse

However, I went down on her and gave her oral sex for a long duration. My tongue was sufficiently inside her and during this, I even found my tongue handsomely in and around her anal area. She orgasmed three times while I was down on her.

From what I remember, she did not have any obvious sores, cuts, in that entire private region.

However, later, I fingered her, as per her request. I did this very adequately...long, hard thrusts, until she orgasmed. When I pulled my fingers out, I had some blood on both my fingers. Not very much, but certainly noticeable. When I panicked, she told me she was on her period.

My mouth, is generally a good clean mouth. My lips though are generally dry, and there's always surface level, cuts  on them.

For all practical purposes, in this discussion, I would like to consider that she was HIV positive. If I'm in for trouble, I would like to see it coming now. I don't want to live under the hope that she may not have had the virus. In reality, there is no way for me to know, but I just want to assume that she did have the virus.

I was having such a great life, I don't know why I've done this to myself. It hasn't even been 24 hours since this unfortunate accident.

Please help me.

Welcome to the HIV forum.

You are overreacting in a big way. If you had followed the advice to read other questions like yours, as suggested in the Disclaimer message, you would have been able to save the posting fee and would have learned that fingering and performing oral sex on women have never been known to transmit HIV.  That your partner had her period makes no significant difference.  Even if we accept your premise that your partner had HIV (but most likely she did not), unless there was unprotected insertion of penis into vagina or rectum, there was no significant risk of transmission.

From a medical or risk assessment perspective, you do not need HIV testing.  However, since you are scared, you probably should do it for reassurance.  Have an antibody test after 6-8 weeks.  In the meantime, don't let this worry you.  There really are no worries here.

Apparently I don't need to say it, but please watch that alcohol problem.  But at least you had only safe sex!

Apologies for intruding on this thread, but I would look to point out a statement (found in many sexual health websites, including CDC) which gives rise to confusion in many users, stating that "HIV is found in blood, semen, vaginal fluid, breast milk and other body fluids containing blood".

This statement alone is rather wide and vague, and gives rise to doubts about such situations as fingering, nipple sucking, kissing, hand/boob jobs and so on.

I've read that the antibody test is only conclusive after at least 3 months, and at 6 months to be completely sure. Since that is the window period.

Do you suggest I should opt for a HIV DNA by PCR? I know that test is not FDA recognized but certain literature suggests that it is quite conclusive at 28 days.

Lastly, the point about her Saliva on my penis, does that pose a serious threat in any way?

Nobody disagrees that CDC's prevention messages, and those of many other governmental health agencies, are overly conservative and can seem vague.  But that is irrelevant.  The science is what it is.  Nobody needs HIV testing later than 3 months; you don't need any more HIV testing at all; and saliva carries no HIV risk, no matter the kind of contact.
