Re-used needles

Ok, i know its really rare at a sex clinic for a doctor to use the same needle, but is it even possible to get hiv from a blood test or is it only blood transfusions. How may known cases are there in the UK of getting of getting infected by a dirty needle from a blood test? I mean it must be pretty easy to make a mistake and use the same needle on someone else
