Risks/Testing/skin symptom?

by worried in toronto, May 30, 2006 12:00AM
Hi there Doctor,

I am a little worried/paranoid about some incidents i have had in the past year and a half since my last HIV test. I met two married women off an internet dating site, and had sex with them. I used condoms every time (twice with one, four times with the others) and i don't recall any breaks, however one time there was an oversized condom used (its a little embarassing) and i changed it after a few minutes as my unit was swimming in it! but it didn't break it was just very loose.

I didn't perform oral on either woman and prior to having sex asked both about their HIV status (which they denied having) and sexual practices with condoms (which they always use). Both were established and career oriented but not happy with marriage.

I received fellatio from both on one occassion and that was it for the oral. Otherwise i fingered both as well.

I did notice a bit of flaking skin (very small) on the side of my nose. I diagnosed myself with Seborrheic Dermatitis and have found online that its rampant in the HIV community which has me extremely worried. I haven't suffered from any of the classic acute symptoms like fever or gland swelling but the Seb Derm has me concerned.

Can you evaluate me for HIV risk Dr.? What are my chances of testing clear on the HIV test? I had the other std tests and they were negative.

thanks a million
Alan

Nobody has ever been proved to catch HIV by vaginal sex with an intact condom, loose or not; nobody has been proved to catch HIV by receiving oral sex; and fingering is zero risk.  Seborrheic dermatitis can occur in HIV infected people, but it's also one of the most common of all skin conditions in HIV negative people.

So I calculate your risk of HIV as zero, assuming you have no other potential exposures you don't describe.  I would have recommended against HIV testing at all.  It will be negative.

Good luck--  HHH, MD
