STD/HIV Gay risk?

STD/HIV Gay risk?

Dr. Handsfield or Dr. Hook:
Thanks for responding to questions.
I wanted to run a scenario from my experience and would like feedback for risk factor(s) if any apply.
I am a gay male, 25 y/o.  On August 6th,2007, I engaged in some unprotected anal sex, whereby I was the recipient. The providing participant “dipped” me briefly for about 45 seconds or so. I thought about HIV/AIDS...and said we should use a condom.  I asked him to put on a condom, and we continued. No less, he never ejaculated. Though I do know that the “dipping” was likely a risk. He also rimmed me, and then performed oral sex on me.
I tested with this Home Access kit at 8 weeks, and 12 weeks. Both were negative.. At the 10th week I got a rapid test done at the Health Dept., which was negative as well.
On Nov. 3rd, I met a new man. Again, I was recipient.  We used condoms and water-based lube. However, since I had not had anal sex since August, it was kind of difficult, and so after about 5-10 minutes I ceased having the anal sex with him. Would I have known if the condom broke? I did see him take it off, and it was to the base of his penis.   I did perform oral sex on this man, but not to completion. He performed oral on me as well, and not to completion. Is that a risk?
If exposed to any bacterial STD from the Nov 3rd incident or August 6th incident, would this have cleared it up?  I was concerned more so about the Aug 6th incident because the man performed oral sex on me after rimming me,…so I did not know if I had some bacterial in the urethra.

I do also want to mention that I was on Zithromax 250 mg at the beginning of Sept for a bronchial infection and also Cipro for a week for a possible UTI.

Questions:
1. How likely is it that I contracted HIV?
2. Would the Levaquin/Zithromax have cured any bacterial STD?
3. Could the UTI that showed high lymphocytes in my urine back in Sept have been a STD?

Sorry about the lengthy post.

Thanks!

First the punch line, then the detail. My guess is that you have not acquired HIV from the exposures you describe and if you had acquired a more typical STD like gonorrhea of chlamydia, some combination of the azithromycin, ciprofloxacin and levaquin you have or are taking will have cured them.  Now for some expansion on these statements.

Having had three negative HIV tests after your August exposure virtually assures you that you do not have HIV from that exposure.  I'm glad to hear that you are practicing safer sex now and using condoms, you should continue to do this.

Regarding oral sex, with or without rimming involved.  These are not exposures in which HIV is effectively transmitted, thus you should not worry regarding these exposures.  On the other hand, infections like gonorrhea and NGU can be transmitted through oral sex.

Having white blood cells in your urine in September may have been a sign of infection/STD.  Usually these cells are while blood cells called PMNs, not lymphocytes so I am a bit confused by the report that you had lymphocytes in your urine in September.
Finally, condoms can break without either partner knowing it.  Condom breakage however is not common and using water based lubricants as you have done reduces this risk of breakage still further.

Hope this helps.  Don't worry about August and keep practicing safer sex.  EWH

I hate to add one more thing..

But is kidney infection or back pain at all related to any STD?

I am currently on Levaquin 500 mg for 3 weeks for a possible kidney infection or prostatitis.
I am getting re-evaluated by a CT scan and having that reviewed by a urologist next week.
Thank you again.

I also did ask the latest partner their HIV status, and he had said negative as well.  He claimed to be tested negative in August.

Thank You Dr. Hook.
Could the back pain that goes around to the right hip side be a cause of some undiagnosed STD?
For example, would gonnorhea (Gonorrhea) or chalmydia have caused symptoms in the rectum if exposed to it?
Since I was on and am taking those antibiotics, would it be a fruitless endeavor to get tested for rectal gonnorea or chlamydia? I have had no symptoms of what people claim as ARS (fever, etc.) And was the Home Access test a valid test to take?   Again, thanks for your prompt response.   I will always "keep the condoms handy" as Dr. H has said in numerous threads.

Would a 6 week Home Access kit provide relief as much as an Oraquick finger test?
