SW Risks?

SW Risks?

While on vacation in a South American country I visited a sex worker. I received unprotected oral sex from her for about 3 minutes and then had vaginal sex which WAS protected. Intercourse lasted about a minute and there was no defect in the condom and it was removed in the usual fashion.

Information online suggests HIV risks from an exposure of this sort are approaching zero, however information from different sources is conflicting. What is the actual risk here?

With regards to other STDs what is an appropriate course of action? Should I forget about it providing I show no symptoms within a few weeks?

For the future, is this kind of sexual behaviour to be considered risky and/or stupid and should I always use a condom for oral sex? Generally, is it a bad idea to visit SWs abroad even when protecting oneself?

As discussed innumerable times on this forum, virtually nobody catches HIV by oral to penile transmission.  Some experts believe such transmission never occurs, and the maximum calculated risk is around 1 transmission for every 20,000 exposures (equivalent to receiving oral sex by an infected partner once a day for 55 years and maybe never getting infected). And that's if the oral partner has HIV.  Condom protected vaginal or anal sex is essentially no risk if the condom remains intact, as it did in this case.

From a risk assessment perspective, you do not need HIV testing -- although you may decide to do it if this reassurance isn't sufficient and you would sleep better knowing you have had a negative test result.

The risks for other STDs aren't quite that low, but in general oral sex is safe sex.  There are very low risks for gonorrhea, nongonococcal urethritis, herpes due to HSV type 1, and syphilis.

There is no definitive answer to your closing question; whether or not it's a "bad idea" is a personal decision.  Obviously, having commercial sex while traveling (or any other time) carries potential STD risks.  However, the chance of infection is low for oral sex and condom-protected vaginal or anal sex.  You can also reduce the risk by being selective in choosing partners, even among commercial sex workers.  In general, high priced escorts probably protect themselves most consistently and probably have lower rates of HIV and most STDs than, for example, brothel workers, street workers, bar pickups, etc.
