What are you going to inform the local health department? They'll tell you don't get any more tattoos and that you could be having a reaction to the ink. But then they can refer you to a mental health professional due to your obsession.

I spoke to the artist as soon as I started getting these symptoms. His told me the same thing you told me. "never heard any incidents". also said he followed universal guidelines.
Since I'm the person whos going through the ordeal ....I still need assurance.  Does the symptoms sound like ...an infection? should I inform the local health department? I do have an appointment in January(1st week) for retesting.

Did you go to the tattoo artist and talk with him/her. There has never been anyone that has contract HIV by tattooing and you won't be the first. If you have a concern with your symptoms see your doctor.

I've heard that chills ,are  indication of infection?  I have chills and neck pain. Its been 4 months since getting the tattoo , is it possible to get this type of symptoms after this long? or is it the later part of the disease? I'm going crazy!!!!!

None of your symptoms sound like ARS symptoms. HIV has nothing to do with Acne. If you are worried just have a test done anytime. Enough time has passed for it to be conclusive.
