Dear Dr.
my story goes like this last year I had to hiv test becuase I changed the place in work in and the law here says that you must test each time and both were -ve. Then in mid december I went to a besuness trip to dubai and I met an Iranian CSW and we went to the hotel together we had sex twice protected but I was drunk and I dont recall if the condom was Latex or another type. she also preformed oral on me with out a protection. The vaginal sex which was protectd I cheaked the condom after each time and it was intact and it was covering my male oragan but the second time due tgo my drunkness my organ relaxed right after i came in her and i contiued stroking in her for a some time then when the shrinking contiued I removed my organ out and the condom was still covering my penis and the spearm was inside it. 6 weeks latter i developed a cold like symptoms for a two weeks and also I had food poisoing for a day oir two. now after more than 2monthjs and a half of the invedent I can feel discomfort in my neak and you can feel a golf ball size bump on the back of my neak on the left side only. what is scaring me is that I had sex with my wife after that insedent.what are my chances Dr.

You do not need to worry.  Here are the facts relevant to your concerns:

1.  The chances that your partner had HIV are very, very low.  Worldwide most CSWs do not have HIV and in Dubai, if anything, the rates of HIV should be lower than in other locations.  
2.  Your genital sex was condom protected.  Condoms work. You did not catch HIV from this exposure.  
3.  There is little if any risk of HIV from receipt of oral sex.  The quoted figure for HIV risk, if one has oral sex with an infected partner is less than 1 in 10,000 and, in my estimation that is too high; many experts feel there is no risk from oral sex.  Neither of us on this site have ever seen or reading the medical literature of a convincing instance in which HIV was passed by oral sex.  
4.  The symptoms you experienced 6 weeks after your exposure are not suggestive of HIV and occurred too late to be ARS.
5.  The bump on the back of your neck may be a lymph node or something else.  Whatever it is, it is not related to HIV.  When lymph nodes swell due to HIV lymph nodes all over the body swell, not just a few.  It could be a bacterial infection or some other process.  You may want to see your doctor about this.

Bottom line, your risk is close to zero.  I hope this is helpful to you.  EWH

thank you sir your assurance. and please allow me to ask three more question would you suspect any other STD that would make these symptoms? the other question is I am planing a test on the 3 months mark which is going to be next week. in your profsional oppion would you advice me to take a rapid test or go for labratory elisa test? and from your vast knoldge in dealing with this illness what are the chancs that me test would be +ve?
I will only post again next week after the test result to keep you informed with my results and thank you again for all the good and kind work you are giving to scared people like me on this site:)

1. Your cold symptoms are not likely to be due to any STD.  Many viral illnesses caught as one goes through life can cause these sorts of symptoms.
2. The performance of the rapid test and the ELISA is similar.  There is a slightly higher risk of a false positive result with the rapid test so I would suggest the ELISA is preferred.
3.  Your chances of having a positive test are very, very low.  I am confident that the test results you receive will show that you do not have HIV.  EWH
