neddle poking pain

Hi, I encountered pain in uethra after unprotected sex with a girl. The back of my thigh also felt tight and painful. Both upper front side of the thigh also felt pain esp the left side. I diagonsis with NGU recently and was prescribed with Zithromax 1G oral. 1st week was ok but the pain in the uethra came back on the 2nd week. Did a smear test( by inserting 1 stick to my uethra) and result is good but i still feel the pain in uethra. Went back 1 week later to the doctor and did another smear test for NGU and found moderate. I also encountered sharp needle poking pain mostly on my leg, thigh, back of knee, toes, fingers.

I had HIV test done 5 weeks after exposure and negative.

Can anyone pls tell me why I have this needle poking pain.
