new possible exposure

Hey guys I had vaginal sexual intercourse (with a condom) but as I was putting the condom on my hand slipped because of all the lubricant in it and I touched the head of my penis with the same hand i was seconds earlier massaging her vagina. and most likely had some vaginal fluids on me....did I put myself at any sort of HIV risk?

2) as i unrolled the condom on my penis i saw a small build up of air in the tip and pressed it firmly as to release the air....is it possible this create micro holes in the condom? ...........thanks a million

One last question please. .i promise. ..for a while she was naked on top of me rubbing against my underwear and then i was on top but still with underwear. . An she was naked does this pose any risk?
