oral sex mistake

oral sex mistake

Hi DR
Ive made a terrible mistake I received unprotected no condom oral sex from a female sex worker.
Before posting this message to you I have read most of the threads on oral sex from the HIV prevention health forums and from the international and HIV prevention medical community. An all the experts from the health forum experts say zero risk from oral sex.
My question to you Dr what are my real risks or odds of HIV from this single exposure?
Do you think STDs test needed ? I have already taken Azithromycin Zitromax 1g yesterday and I havent had symptoms apart from a cough now and again.
I really want to hear it from you Dr.

Welcome to the fourm.  Thanks for researching your question.  But I have to say I would have thought that, doing so, you would have learned what I would say.  Indeed, in response to this same question on the HIV community forum, Lizzie Lou correctly quoted a number of replies to similar questions by both me and Dr. Hook.

The best available data -- not great data, but still the best estimate -- come from a study published by CDC, based on interviewing newly infected persons about their sexual exposures, and with knowledge of the likely proportion of partners who might have had HIV.  That analysis suggested that the likelihood of oral to penile transmission by fellatio, if the oral partner has HIV, is around 1 in 20,000.  That's equivalent to receiving BJs by infected partners once daily for 55 years before transmission might be considered likely.  In your case, the chance you caught HIV is even lower, since the vast majority of female sex workers in the US do not have HIV.

Bottom line:  you can consider this a zero risk event with respect to HIV.

Hi Dr
Me again, thanks for your comforting message
Do you think I should get tested for other STDs? I was advised to take 1 gr of Azithromycin just in case before testing. and that would clear most of STDs
Whats your advise on STDs for this exposure
Regards.

Having taken azithromycin (which I would not have recommended), there is no point in testing for most STDs; by preventing all bacterial STDs, testing for them would not be valid.  Even without treatment, in the absence of symptoms like penile sores or discharge, I would not recommend testing after a single exposure of this sort.
