as up to 40% of people who have been infected get no symptoms at all, the only way to be sure is to take a test, test at 3 months post exposure for a conclusive result.
As Mitch rightly said, symptoms do not mean much when it comes to HIV, because first, they resemble with those due to other illnesses second, many actually infected people remain totally asymptomatic.

No ! Only specific antibody test determines the HIV status. And this test is ordered by the doctor if he is aware of such kind of exposure, other than routine initial pregnancy tests and blood screening for transfusion (this test is not an antibody test but a different one called NAT)
