Having all medical records transferred to PCP

This isn't exactly a direct question about insurance. My question is more related to visiting care centers elsewhere and needing to get lab info and diagnosis info to my primary care physician.

I have spent the past 3 years at a college far from home and then at an internship for 6 months that was also far from home. At college I found out I was allergic to pseudoephedrine, and had blood tests done to see if I had hypothyroidism. During the internship i visited two different urgent care places and also had labwork done (Western blot) at a separate place when I was suffering from Lyme Disease. My primary care physician likely does not have any of this information and I'd like it to be in my medical records at his office. I also would like a personal copy of my bloodwork from college and the Western blot results (I have my own medical records in my house).

How would I go about doing this? I'm assuming I could call up my college and the urgent care locations and request them to fax information in my files to my PCP (and I'm assuming this may cost me some fees). I don't have a personal fax machine, can I request my lab results to be mailed to me? And what are the general fees for such requests?

Dear salix288,

To get a copy of your medical records you will have to submit a request in writing to your past providers (such as your college and the urgent care locations, labs) to fax your records to your PCP directly. In your signed letter of request you should specify that you are requesting and authorizing the provider (mention provider’s name) to fax all your existing records to your PCP (also specify the name & fax number of your PCP) for the purpose of treatment. Since they will not be making copies or mailing anything they should only charge you for their & fax services!

You can try calling them but under the HIPAA law they are obligated to have your request on file in case of a future audit!
