liver transplant, with access plus?

my daughter has hep c, advanced liver disease....to  much scar tissue to heal, and hepatic encephalopathy.
i am trying to find a specialist in the philadelphia area that would care for her, and put her on a transplant list.
is this possible with only madicare's access plus?
please, am i just wasting my time?  she has been "clean" for almost a year.  is going to mental health.  had a tox screen that was clear of unperscribed drugs.
she is having so much pain from lactulose that she refuses to take it any longer.  it's been 9 moths of agony for her on this drug.  and she's not that much better anyway.
we really need to find a good doctor to care for her.  i won't go into the horrible care she had been receiving.
please who do i call regarding this urgent problem?

Dear DesparateMom939,

It is my experience PA State offers “general case management” and “beneficiary assistance activities and specialized DM activities” through Access Plus. These services are offered through DM & PCCM by mostly nurses but sometimes clinicians also who are specially trained and certified in particular areas (i.e. asthma, diabetes, chronic obstructive pulmonary disease, coronary artery disease, and congestive heart failure).
The DM specialists will work with targeted clients mainly through telephonic contacts, but there will be some face-to-face contact with beneficiaries who have special needs. I recommend reaching out to the DM or case manager in charge of her case. Her PCP can also help you determine the right course of action.
