Short question, but it might be too long and too complicated to answer well here!  I'll take a stab at it, but I'm not a dietician.

You are right in that some foods can affect how well your blood clots, and can affect certain blood tests that you must monitor if you are on blood thinners.  

Basic suggestions:

Limit your consumption of high Vitamin K vegetables to a few ounces per day.
Vegetables that are highest in Vitamin K include spinach, kale, parsley, swiss chard, Brussels sprouts, lettuce, broccoli and turnip greens. If you want to eat larger amounts, be consistent with how much you eat day to day, so that your medication can be monitored correctly via your blood tests.

Try to stick with  vegetables low in Vitamin K such as corn, carrots, cauliflower, eggplant, onions, green beans, peppers and tomato.

Eat other foods low in Vitamin K, including meats, fruits, grains and dairy.

Use oils that are low in Vitamin K such as corn oil, sunflower oil, peanut oil and sesame oil. Canola oil and soybean oil have the highest Vitamin K content.

As for type 2 diabetes...are you on medication or insulin?  Obviously, you need to limit your intake of sugars, highly processed foods, fruits, juices, etc.  You may not need to keep them out of your diet completely, but limit them as your doctor advises.

I feel you really should ask your doctor for a consult with a registered dietician.  I think what you are asking for is too detailed and complicated to answer here.

thank you its a start for me, i will be seeing my doctor next and hope to ask why my p.t test dose not stay at level 3. and gose down to level one witch is bad
