rerouting after open heart surgery

I have heart disease. I have 11 stents, and had  open heart surgery 18 months ago. I just recently had to have another heart cath, and was told I have lost some of the graft, and it didn't heal, so now it is rerouting.  What exactly does that mean? Is that good, or bad? I go back to the doctor next week, but would like  to know what  can be done if anything. I've never had to take nitro, but now I am taking it for chest pain when needed. I have been trying to find information on the rerouting so I will have an idea of what it means.
