100% blocked coronary artery- treatment? dangers? what happens next?

100% blocked coronary artery- treatment? dangers? what happens next?

I’m a 59 year old man from the South Jersey region. I got my first Cypher drug-eluting stent on November 30, 2005.  At that time, I had another coronary artery that was partially blocked but did not require a stent as it was just above the “cut-off line”- it was 60 to 70% blocked. I went to my cardiologist this past April, and took a stress test. My doc told me that my heart function dropped from 57% to 45% since my last stress test in 2006 and that I now needed a stent in the artery that they had decided back in 05 to wait on.  I went to get my second cardiac cath done at the beginning of this May. I found out then that the un-stented artery is now 100% blocked.
Now, I’m going back to the hospital next week to get a third cardiac cath and hopefully get that second stent. However, I just found out yesterday that the doctor performing the cardiac cath/stent procedure may not actually be able to put a stent in because of some “lesion.”  I think what this means is that if the artery has calcified too much, the doctor can’t break through the plaque. If I can't get the stent, how dangerous is it to leave the artery as is—100% blocked?  Do I have to get bypass? (that seems dangerous in and of itself) Can they even put a stent in an artery that’s 100% blocked—and if not, what does happen next?

Thanks for the post.

It's a complex issue and not an easy one to answer without having all of your information available. The main reason to place a stent is to relieve anginal symptoms. Other potential benefits of placing a stent or performing bypass would be to alleviate what is called silent ischemia. This means areas of the heart that are not receiving blood due to blockages. In the first situation, there is clear benefit of placing stents, in the last it's more of a gray area and would depend on the location and the difficulty of performing the procedure.  In the case of a 100%  blocked artery, or total occlusion the procedure is by definition more difficult.

Either way, since you have significant coronary disease, the most important thing is very aggressive control of all of your cardiac risk factors.

I also have a large inguinal hernia that needs to be repaired within 6 months or so.  Can I have the surgery if they can't stent my 100% blocked artery, or do I risk a heart attack on the operating table?

Call Robert Wood Johnson Hospital (NJ) and get the name of a good interventional cardiologist. That is a very good and well-known hospital in NJ.  You need an MD that will manage yourcare and not let so much time go by.  Weren't you feeling symptoms?  Why did your cardiologist wait so long to recheck the 50-60% blocked artery?  Plaque can build up very quickly.

I am a 52 year old male.  My family has a history of heart diease.  I have tried to limit all risk factors except for herdity.  I was in good shape until last month when I started to feel a burning sensation.  I went to gastro doctor and had a endoscopy and colonscopy done.  Both were negative.  I then went to a heart doctor.  He did a nuclear stree test and found something abnormal.  He scheduled a heart cath and found one of my 6 arties had a 50% blockage. This 50% blocked artery was off the main artery.  The only thing he recommended was to increase my liptor (liptor) from once a day to twice a day since my LDL was higher than my HDL even though my overall Chro was 144.  I continue to have a burning sensation in my stomack and get very fatigued easly just by walking  He said to only come back if I have chest pains.  Why didn't he recommend a stent ?  This is like walking around with a time-bomb ....

1. Assume a Horse Riding Stance.
2. Raise your hands to chest height - palms facing inward toward the body and fingers pointing to each other (imagine holding a large ball on your chest).
3. Cross both arms in front of the chest with the right arm on the outside of the left arm.
4. Curl the middle, ring and little finger of the left hand, but keep the forefinger extended forward and the thumb pointed upward.
5. Inhale as you open your left arm horizontally at shoulder height to the left. Push and stretch away your hand palm and index finger (at right-angles to your arm). Keep the vision of the eyes on the left hand throughout the movement. Image that your left palm is pressing flat against the wood of an archer's bow.
6. Simultaneously, and corresponding to the movement of the left hand, clench the right hand into a loose fist and pull it horizontally at shoulder height to the right. Imagine you are drawing back a taut bowstring by leading with the right elbow and keeping the back of the fist facing outward. Hold the position for one second.
7. Exhale as you release botb fists and return the arms to a crossed position in front of the chest (left arm on the outside of the right arm).
8. Repeat the exercise to the right side of the body.
