I am a known case of apical HCM for 2 years. I'm 27 years old and other members of my family don't have any problem. I used to be a professional basketball player too but I quit 5 years ago before having any symptoms. I was asymptomatic till 5 months ago when I started to experience frequent PVCs which I feel aggravated with emotional stress and alleviated with Propranolol. For the previous 2 weeks the frequency of PVCs has increased so that sometimes they come 4-5 times a minute. Once I guess I had a run of VT with associated light headedness but fortunately no syncope. The pressure gradient of 60 mmHg in my LV seems to be mid cavitary and my question is what is the next step for me? Are the PVCs predictors of the sudden cardiac death? Do I need an ICD? I'm going to have a Holter ECG monitoring tomorrow and I will send you the results but please say something encouraging because I feel terrible right now and I have the feeling I'm going to die very soon.

The runs of PVC's (VT), especially with associated presyncope, are predictive of sudden cardiac death in patient with HCM.  Based on that alone you qualify for and should have an ICD put in relatively soon regardless of what the Holter shows.
