Not to second guess the answers of a doctor but I had the EXACT same problems.  Saw cardiologists and numerous other specialists.  The first doctor I saw was my internist who said the problem was simple but did send me to a gstroenterologist to see how bad it was.  He did an endoscopy and told me I had a VERY small "ulcer" at that very spot caused by acid reflux at night.  His saying it was so tiny I didn't believe it could cause me all that pain and suffering.  So I did not follow his instructions.  I went from doctor to doctor discovering everything else was fine. Wasted thousands of dollars.

Only then did I slightly elevate the top of the bed so it pointed down maybe 15 degrees plus took a Prilosec tablet in the morning.  And tried to keep acids out of my diet plus not eat after 6 PM.

Within a week - problem fixed.  Chest pain gone.  shoulder pain gone.    Often it is the simplest answer.

Good for you!  But you should always second guess a doctor, they aren't always right. My husband went thru the same thing, taking a prescription medication for GERD, which didn't help.  I bought him Prilosec, and all is well!
