Back Pain and Palpitations/Skipped Beats weeks after taking 1 Elavil

I was diagnosed with MVP four years ago, I only had palpitations and no regurgitation and pretty much no pain.  A doctor prescribed elavil for me about a month ago, it made the skipped beats and palpitations much more pronounced and increased my tremors.  I developed a cough and pain just right of my left shoulderblade.  The symptoms continued for a week so I talked to my cardiologist and had an EKG that showed a skipped beat.  He took some blood, chest xray, stress test, and did an echo.  He said my heart looks strong and that I should avoid taking elavil because it can affect people with MVP.  

I continued to have the pronounced skipped beats and cough and back pain and am also dizzy and fatigued (heavy arms and legs)now, I also had chills but no fever.  I have had some prostate problems (chronic infection), and the same doctor that prescribed the elavil prescribed levaquin for the prostatitis.  This is about 2 weeks after seeing the cardiologist.  About 12 hours after taking the levaquin, my cough decreased significantly, the skipped beats are much less pronounced, and the dizziness is a bit less.  Its been about a month since I took the elavil and I have been on the levaquin a week.  I am feeling nauseous and have a blood taste in my throat.  I still have the pain in my upper back, cough sometimes and am dizzy and fatigued. Im worried that it could be endocarditis.  
My questions:

1- Does endocarditis include back pain?
2- Do you think the bloody taste/nausea is heart related?
3- Cardiologist said talk to primary physician, how should I approach this with him?
