Bypass, CHF, Arthritis

Hi,

I'm not even sure why I'm posting except to maybe get some reassurance and get this out of my head.  I'm very scared right now for my mother and don't know what to do.

My Mother is 61 years old.  She has a lengthy medical history, but here are the important ones.
History of obesity (5'4", 240 lbs for years, current weight is 196 lbs).
-Diagnosed with hypertension mid-1980's.  Controlled for years via medication.
-Diagnosed with diabetes in early 1990's; controlled currently with Avandamet and diet. Blood glucose level hardly ever above 120.
-Diagnosed with severe, diffuse, degenerative psoriatic arthritis with mild osteo arthritis. Severely effects back, hips, knees and all other joints (in that order of severity).  Ambulates with a rolling walker until recenlty (read further below). Had to retire Feb 2007, and is currently on SS disability.
-Diagnosed with osteoperosis (sp?) several years ago.
-Jan 2006 had a silent MI, and only symptom was shortness of breath, which was attributed to pneumonia, and was treated as such. Doctor ordered electrocardiogram just to make sure.
-Electrocardiogram showed inverted T-waves (I think that's what they called it), and ordered stress test.
-She had stress test (via medication, cuz can't do the treadmill) which showed abnormalities (don't remember what they were).  Doctor ordered cardiac catheterization.
-Cardiac Cath showed 85 to 90% eclusion of 3 valves and 100% eclusion of one valve.
-June 29th 2006, had triple coronary bypass surgery at St. Michael's in Newark, NJ.
-Developed high fever for a week or two, but eventually went close to normal, and she was sent home with no cardiac rehabilitation at all (a therapist showed up to the house one time).  Tried going back to work in November, but was unable to continue and retired Feb 2007 (actually she was "asked to leave").
-She has PVC's all the time (her heart skips a beat every few seconds).
-November 2006, one of her bypasses closed up and was discovered in later cardiac cath procedure.
-Heart operating at 45%, according to cardiologist, 55% is normal (not sure what she called it).

Christmas Eve 2007, she fell down a couple of stairs, breaking her left ulna on forearm, and right shoulder at the head of the humurous.  They sent her home with an arm brace on one arm and a sling on the other and told to see orthopedist.  She couldn't walk upstairs to her bedroom, because she uses her arms for support, so ambulance crew took her home and put her in bed.  December 26th, had to call 911 again to get her out of the house to the ER.
She had surgery on the right shoulder last Thursday (1/3/2007) and had a rod and a few screws put in.  Slllooooowly getting use of her arms back.  She can't walk any more without someone basically carrying her on her feet, because her legs are so deconditioned from not walking for a week.  She can pivot and walk MAYBE 3 steps.  She could barely walk before, and now I'm worried if she'll ever walk again.

She has been running a 101.4 fever since before the operation except for 2 days before surgery.  Infectious Disease doctor can't find anything wrong so far.  Had electrocardiogram done today and will see what's up.  She was diagnosed with congestive heart failure the day before yesterday, which she never had before.  

I feel like my Mother is dying, and nobody can figure out what's going on.  She's very scared and I'm very scared.  I'm terrified that our insurance carrier is not going to pay for anything, because I've already gotten 3 letters from them (I was cc'd on them, but was directed to the hospital) denying all claims so far, but also stating that we are not responsible for incurred charges.  Even if she gets through this, she still can't walk, and our house is all stairs.  She's going to be trapped upstairs, and it's unknown whether she'll even be able to make it to the bathroom on her own.

Is there anything that we're missing, or am I totally exaggerating what I see happening?  Will insurance usually pay for physical rehab at all, or is she out of luck?  If so, how much rehab is normal?  How much does physical rehab cost out of pocket in NJ?  I can't find info anywhere on-line.

I'm sorry for taking up so much time without a definitive question, but I needed to get this out of my head before I have a nervous breakdown.
