Can feel pulsation at 2nd intercostal space when leaning forward

I can feel a pulsation at that location which is 3 inches below the collar bone and slightly to the left against my hand but only when leaning forward and in the right position I cannot feel it when standing normal or lying down. .  I don't have health insurance and was wanting to get some opinions on what this could be.  I first noticed this six years ago and it has not worsened.  Around the time period that I first noticed this  I had a few ekgs, cardiac blood work up and a chest x Ray.  Everything was normal.  Also had a holter monitor that showed nothing serious.  I found something online suggesting pulmonary hypertension but when it talks about doing the Tess the patients are laying down on their back.  

Maybe someone can help me.  Thanks !
