Cardiomyopthy and Pregnancy

My name is Samantha.I have had two pregnancies,in both of them there were issues with High Blood Pressure. In the latter pregnancy, I suffered a cardiomopthy.  I am now 38 years old, my husband and I are considering have another child.  I know that there are some risk in all prenancies, but I would like to know what steps should be taken prior to getting pregnant again, or is this even something that should take place. I love my family and want to be around to see the fruit of my labor become adults and began families of their own.  I know that this decision is really between my husband and myself, but I want some sound medical advice. Thank you.

It's my understanding that even if there is the smallest risk that there was a pregnancy related cardiomyopathy there should never be another attempt at a pregnancy.  Both of my cardiologists have said they would say an absolute No if someone with a PPCM wanted advice on additional pregnancies.  Of course it is your own decision, but please talk to a cardiologist for solid advice.  Also, there are forums for people who have had PPCM.  You might wanna check that out also.  Good luck

I also had PPCM with my second pregnancy.  The risk of recurrence is around 20%, and that is only if you have recovered full function and heart size.  Your EF should be over 50%, your Fractional Shortening should be over 30% and your heart chamber size should be completely normal before even considering another pregnancy.  A test called a Dobutamine stress echo can measure your contractile reserve, and this is an extremely valuable test in determining who is a good candidate for a post PPCM pregnancy.  Some women do it with success, though some obviously will relapse.  Relapses can be managed, but it is best to see someone who specializes in pregnancy-related cardiomyopathy as a cardiologist and also to have a high risk OB on board for a subsequent pregnancy.

Myself, I've decided that since I have two children, I'm not going to risk my health for another pregnancy.  That is a very personal decision each woman must make on her own, with advice from her own doctor about her specific condition. The good news is that post-PPCM pregnancies aren't the doom and gloom guaranteed relapse once thought...but there is still a SUBSTANTIAL risk.  Women can suffer lifelong disability or die from PPCM, and if a relapse occurs, it typically comes with a permanent setback (i.e. lower EF even with a second recovery, etc.).
