Chest Pain and Lightheadness

I am a 41yr old male who has been diagnosed with high blood pressure, high cholesterol and triglycerides and boarderline high sugar.  My cholesterol is 232, HDL is 28, triglycerides are 502 (down from over 1,000 while using Tricor), and fasting sugar at 115.  Recently, I have been experiencing dull chest pain, which comes and goes randomly, but I feel it mostly at night while in bed.  Additionally, I often feel like I'm going to faint, losing my balance.  This happens on a daily basis.  I had a stress test two years ago, and everything was fine.  However, these recent bouts of chest pain and lightheadedness really has me worried. What could be causing the way I'm feeling?
