Chest pains, fatigue, numbness, Highly variable BP

I am a 38 yr old male. A somker - 20 a day. Weight - 85Kg/187Ibs. Height 5ft 9.

9 months ago I started to suffer from bouts of extreme exhaustion, which soon became the daily norm, no matter how little exercise or how much sleep I got. I have had painful joints and muscles every day since shortly after this began. I suffer with severe daily headaches and eye pain, and 8 months back had several painful episodes of monoclar blindness lasting several hours. Around a month after the exhaustion began I would get intermittent severe squeezing pains in my upper left chest, often early in the morning that would wake me. I have had numerous dizzy spells an felt faint, and had numerous episodes of numbness and burning feelings in my arms and hands. Around 3 months ago the chest pains became far more frequent and centrally located - like a heavy weight on my sternum. The pain is there all day now. It often radiates out through the pectoral region on both sides (though worse on the left) through the armpit region and out through the shoulders and upper back where it often feels like a stabbing pain. I have also had a very tender abdomen for last few months and a lot of stomach upsets. over the last few weeks the chest pain has been there constantly and the numbness in my fingers is there most of the day, I am also getting very bad shooting pains through my wrists and hands and fingers and have had (less frequently) pins and needles in my feet and toes, and numbness and burning in m tongue and lips. I also have developed a daily dull ache in my right testicle and often get a deep muscle burning sensation in my arms. My BP is highly variabe. On my left arm it is reasonably consistent at around 125/75 dependent on time of day, but my right arm shows huge variation, rising as high as 180/110. Often when I take a reading there is a systolic difference of at least 15 between left and right. this morning my left gave me 117/69 and 126/75, my right however gave 161/100 and 149/96.

My local Dr has been quite dismissive of my concerns and put joint pains down to fibromyalgia and chest pain to costochondritis. My own research suggests that a difference between right and left BP could be a serious cardiac problem. I have had a number of blood tests, way back when the exhaustion started, but before the chest pains and numbness became so severe, that were ok, and in January a visit to the emergency room, where a chest x ray, ECG and bloods were ok. However these tests are not sensitive for all potential cardiac problems. Online symptom checkers seem to point towards autoimmune diseases with vasculitis, more often than not polyarteritis nodosa.

My doctor will not refer me to cardiology on our National Health scheme, do you think I should pay to see one privately? Surely an ultrasound echocardiogram is needed at the very least?

I forgot to mention I have had several bouts of raynauds in both hands in fairly mild temperatures over last 3 months
