It sounds like you have congestive heart failure which is manifest by an audible "crackling" in the lungs when fluid accumulates in the interstitial spaces between the air sacs of the lungs. Your medications sound appropriate, but certainly tricuspid valve regurgitation and a residual VSD (possibly created from the pacemaker insertion) could be causing you to have worsening congestive heart failure. My suggestion to you is to see your cardiologist soon and have another echo performed. Otherwise, I can't provide you with more specific information. Good luck!
