Dilated Aortic Root with Bicuspid Aortic Valve

Dilated Aortic Root with Bicuspid Aortic Valve

Male, 51 years old, Competitive runner for over 30 years. 30+marathons, 4 time Boston Marathon finisher, 10+ ultramarathons. Bicuspid Aortic Valve.
Dilated Aortic Root of 4.1 - 4.3 cm. for at least the past 6 years. Echocardiograms every year plus a max stress test echo in 2005.  Recent CT scan indicates 4.1 cm. Normally working bi-cuspid valve. Have followed the valve issue for much of my adult life, but my new cardiologist was the one to bring the root issue to my attention a few months ago.
Are my competitive running and cycling days finished?
Thank you

I am afraid that might be true in your case.  As you know, it is not the aortic valve that the physicians are concerned about, but rather the aortic root which has a higher risk of dissection in a patient such as yourself.  You may know that dissections may be fatal, especially type A, which you are at highest risk for.  You should probably already be on beta blockers to decrease your risk of dissection.  Of course there really arent' any studies that looked at this, it is more of a recommendation based on professional opinion.

Thank you for your time and for the reply.
I was on a beta blocker for 3 days and felt terrible.
My resting pulse rate is near 40bpm, so my reaction was somewhat expected.
Two further questions though.
The excercise recommendations for me were max. 70% from a 2005 study.
Based on my past history, would short periods above that limit be possible?
Also, any other tests that might be recommended?

You would really have to talk to a vascular medicine specialist who deals with aortic disease to get their opinion regarding this.

No further testing is recommended at this point.
