Echo results, for blood pressure question

by Mieke, Apr 18, 2004 12:00AM
This is Mieke to CCF-M.D.-RCJ, thank you for answering my questions yesterday.  With regard to my concern about my blood pressure not rising with my heart rate during exercise you said you would like to have seen an echo report.  I had an echo done in January this year and the report said:

"The mitral valve appearance was consistent with mild bileaflet prolapse and there was a thin jet of posteiorly directed mitral regurgitation (1+). Left atrium was at the upper limit of normal size having a cross sectional area of 20cm2."

Everything else in the report appeared normal.  Does this help in you being able to tell me if you think the blood pressure not rising is of any concern? or why you think it does not rise as it should?

Thank you for your time.

mieke,

I've read your post from yesterday.  The question RCJ was referring to was what happens to the mitral regurgitation in the setting of exercise. If it doesn't increase or other abnormalities develop (ie cavity obliteration) then as stated it isnt necessarily common, but can happen.

The pressure not rising is not of concern unless conditions such as above are developing. The reasons why some people have a blunted response in the setting of what appears to be normal cardiovascular function is unclear, but probably not of great significance.

I dont understand the echo findings.  I had an echo that said normal left and right atrial sizes .  Normal right ventriclle size.  Then it said mild left ventricular chamber dialation it was 5.7.  On this board I saw were a 6 was normal.  Then it said Moderate to severe mitral valve prolapse with mild reg. Normal aortic root and valve with mild aortic reg.  I asked the dr about the aortic valve he said it was normal.  A lot of people have it nothing to be worried about.  Then I read on here its not normal.  They did a heart cath and it came back mitral valve prolapse with no noted reg.  I had an ep study because they saw a 4 beat run of vtach on my holter report and a 3 beat run.  I have spels of bigeminy which drive me crazy.  He ablated one place in my left ventricle but couldnt induce anything bad and couldnt find the place causing the bigeminy.  My ef fraction was at 60%.  I got a copy of my records and one of the drs at the clinic had written that I would probably need a valve replacement.  I do have thickening of both leaflets.  However the other two drs say it may never progress any futher its hard to say.  My ep dr says its no big deal but all of this has bed ridden me for the most part.  Anyone have any thoughts?

You sound very normal...don't worry and carry on normally, follow up with your Doctor.  Nothing in your data would worry me at all...wish I had your data!!  Yes, ectopy can be anxiety producing for some...but I have so much that I have grown accustomed to them and give them no consideration unless I get dizzy etc.  so good luck.

How bad is your ectopy?  I have had several holter and 30 day event monitors I think the most they found was 972 most of them in bigeminal cycles.

Practically all of the pvc's I experience are the kind that make me lightheaded/fainty feeling followed immediately with a burst of anxiety.  When pvc's happen close together there is lots of the anxiety, fainty, lightheaded feelings and makes me feel terribly baaaaaaad.
