What effects, if any, are there with a + 4 leak and 45-50% EF?

Does the reduced blood flow/volume have effects 'downstream' at cellular level?

Any effects on Joints, bones, tendons, muscles?

Generally the heart is able to maintain enough forward output early on so there is no damage to the body.  If left untreated heart failure ensues with all the syndrome that goes along with that.
