Elavated Blood Enzymes

by BlackHart, Oct 30, 2004 12:00AM
I am a 38 yoa male with MS, currently on betaseron, baclofen, and zonegran for it. Also on Nexium, 81 mg aspirin, vitamin c,  In the last 2 month I have had episoded of sever chest pains that normally start as burning in the throat, proceed down to the chest, than turns to pain, and it hits my left arm which usually goes numb.  This has put me in the ER twice, with each time the blood work showing elavated trops, CKMB.  The docs have done a stress test than scanned my chest with a machine that looked like a big circle, heart catherization, pulmonary catscan, and a carotid doppler each of those test have shown no reason for me to be having these heart attack episodes.  My cardiologist has placed me on Norvasc, he think my arties may be spasming but since being on that med I have had an attack that put me in the ER, with what appeared as having a seizure as well, I was not able to talk clearly ( stuttered ) for about 1/2 hour and my left arm and leg were jerky. My stress test on the first episode showed no signs of infarction or ischemia, but I did have a somewhat depressed left ventricular ejection fraction at 45%.  The docs are not sure why my hear is releasing the enzymes if every test they have done comes back negative for a heart episode.  I have been giving a GI cocktail, and nitro pill which both did nothing to help with the chest pain while in the ER.  Just curious, have you heard of something like this before? can my MS be causing my heart or arties to spasm in essence making them release these enzymes?

Thank you in advance for your assistance.

Blackhart,

Thanks for the post. Im sorry youve had such a difficult time.

If there is serologic evidence of myocardial enzyme release -- other causes if your catheterization and CT were negaive would be spasm of the arteries in the heart, myocarditis, or elevations from renal disease.

WIth the episodic nature of your attacks, vasospasm would definitely be a possibility and treatment with Norvasc definitely may help.

Beyond this, I woul make sure you continue your evaluation with a cardiologist to follow your elevations in the enzymes.

This is a real interesting scenario. I am sorry you are going through this. I was real interested in the doctor's response.

I was wondering if the CKMB enzymes that were checked are specific only for Cardiac or if other muscles in the body could be releasing them into the body. The doctor answered that by stating that the kidney could be a possibilty. I wonder if the Heart would release those enzymes just from Coronary spasms?

Let me know how you make out and if the Norvasc helps. If it does then it probably was Coronary spasms. Best of luck and I hope you don't have any more problems!

hey,

sorry to hear about your trouble. I would have liked the answer from the doctor if your MS could play a roll in that, maybe he can still give us that information. i, myself been diagnost with ms and have some hearttrouble aswell.

I hope the medication will help you, good luck.

There really isnt any definite connection between cardiac problems and MS in the literature. There have been some ancedotal reports of people developing some element of left ventricular dysfuction with mitoxantrone therapy, but not too much else.

If you are taking supplements or other natural treatments for your MS this could also be a potential cause. The episodic nature of your symtoms still would suggest something more like vasospasm.

good luck
