Extreme high blood presure

Hi, I have a brother inlaw that has extreme high blood presure,
I need a few theorys,  Doctors have not been able to lower it.
This gentleman has been prescribed and taking these medications.
Losartan dot 50mg,  Klor-con 20mg,  Hydrochlorot 25mg,  Bystolic 20mg,
Metaformin 1000mg,  Asperin.  taking them twice a day.
Suffer from sugar diabetes too.   Please advise.

The more information you have the better.  The pieces you have given are good enough.  Yes, I would say his bp is not under control.  If he is having reading such as that every day or often he should be hospitalized and a specialty consulted, such a Nephrologist and his bp meds sorted out.  With readings that high usually I am giving my patients bp meds via IV.  In my opinion, sounds like a possible kidney issue.

Probably so-called secondary hypertension. Hard for me as a patient to guess, but in the setting of high sugar, pheocromocytoma is a possible condition? Other hormonal problems are also possible. Narrow arteries to the kidneys are possible. The first conditions van be ruled out with blood tests. Last one with ultrasound. It is really uncommon that heart diseases or stress cause blood pressure at that level.

Sorry for not posting that part.    Average  BP is 235/118
I am not sure if  Metformin is controlling his sugar.
Keep it in mind,  I'm worried for my bother in law,  His name
is ysidro. He's the one with all the information.

Can you give me the exact BP numbers?  I need you to be more specific.  

Secondly, are his blood sugars controlled with the Metformin?  

Has his kidney function been checked?  Diabetics with uncontrolled hypertension often have kidney issues.

I would certainly see another doctor, preferrably a cardiologist, it seems to me that there should be something that would help. There are many blood pressure pills on the market, and sometimes, even, IV drips of blood pressure meds to get a large dose in the patient to help get the pressure lowered quickly, then, they take the pill version.
