Have many ECGs, 1 Echo, stress test and Angio but still having pain in my left arm

Hi,
I am 31 year old Man.

2 weeks before suddenly I feel pain in my back chest and then in the morning 4AM there was shortness of breath and lot of burping and pain in the chest. I went to emergency there they had ECG and Troponin test both were normal. but very next day again I was having pressure in chest and pain in back chest and arm as well. I went to a cardiologist, he did ECG, Echo and stress treadmill test. All were normal except stress test. I did not have any pain but Dr told me there are some variations in the reading so he want to do Angio to make sure it not cardiac. Next day I had Angiography and it was normal and my Dr said it just a muscular pain.

But after two day the pain was again there. I am taking muscle relaxant, now there is no pain in the chest but it is there in the left arm and shoulder.

My question is what was the cause of positive stress test. If angio is normal what is causing this pain. I also feel dizziness sometimes.?
Does angio rollout the CAD?

Your pain may be absent at night because your head and neck are properly supported so that there is no strain on them, nor pressure on the nerves.

Your doctor did the angio and concluded that your heart was healthy after that.  You have had *extremely* thorough testing of your heart, and you can and should believe the results you have gotten.

As I mentioned, your finger pain is not heart-related.  It is most probably caused by some irritation of the 7th Cervical nerve, which is in your neck.  An orthopedist or neurologist will be able to help you with that.

If you cannot accept these facts, then your next best bet is to see a counselor to find out why you continue to be fixated on your heart.  This is called a 'cardiac neurosis,' which you can google.  It can be treated.

Hi Again,
I have been taking muscle relaxant for 15 days now but no improvement. The pain in the back, arm, pressure in the chest and now pain in the left side teeth and jaw is also there feel tired and pain in legs as well, that is disturbing me. Dr said it is due to muscular spasm, but there should have been some improvement in after taking medicine.

what should I do now?

Have your doctors told you that you do not have a heart problem?

If they have, then--if I were you--I would try to believe it and get on with my life.  If I found myself still overcome with anxiety about my heart, I would seek counseling for emotional help with this problem.

Thanks for response, I will stop smoking right away. But are you of the same view that I do not have heart problem and do not need any further investigation????

These days, troponin is considered much more reliable than cpk for diagnosis.

A lot of things can cause elevated cpk, things like injury, and your angiography could be considered a mild injury. Usually, though, the cpk levels return to normal pretty quickly.

Some healthy people and some ethnic groups normally run high levels, and in such cases, the elevation does not seem to predict anything bad.  You presumably do not know what your cpk was before all this started, so you might just be in such a group.

However, I can tell you this:  smoking harms the lining of blood vessels, and it reduces the amount of oxygen that the heart needs.

If you are concerned about the health of your heart, the essential thing you must do is stop smoking right now.  That is the bottom line.  It is actually pointless to worry about blood tests as long as cigarettes are part of your life, for you are damaging your cardiovascular system with every one you light up.

Hi Achillea,

Sorry to disturb you again, I went to doctor for cervical check up, he did the neck X-ray and find there is some issue in the X-ray and was saying the pain could be due to this. But in addition to this he di my CPK test and that was not normal, test showed value 300 greater than normal range. Doctor explain it could be due to recent Angiography and could be due to some other reason. But I am really worried again why CPK values on higher side. and its been almost 3 weeks after  Angio. On the day of Angio CPK was done 3 time and it was normal.

I missed one thing in my earlier post that I am a smoker and smoke 8,10 cigrates for 8 years.

Pls advise is this something to worry about or related to heart or just could be some other reason of high CPK???

Yes this nail pain is the main cause of worries and another point is I feel no pain during night. I would like to repeat my stress test was positive then Doc decide to do Angio. Still I can believe that its not heart related after having positive stress test and these pains????

You've had an EKG, tests for troponin, echo, stress, and angiography, and your heart has shown that it is healthy.  I think you can accept this great array of testing as valid.

You wonder if you are suffering from anxiety, and my general impression from what you write is, yes, you are suffering from that.  Whenever people mention burping in connection with worries about serious heart disorders, even after testing, that, strangely enough, is a red flag for anxiety rather than a cardiac problem.

However, here is the key:  You have pain that involves your shoulder, your arm, your hand, and--most notably--your middle fingers.

You are describing an irritated cervical nerve--a nerve in your neck.  The fact that your middle fingers are involved indicates the the nerve called C7 is involved.  You may have a herniated cervical disk, or you may have something called cervical radiculopathy.  You can google both these terms, and you can watch this video:

This pain complex has nothing to do with the heart.  It can usually be treated fairly successfully without surgery

If, after you see a spinal specialist, you still are fixated on your heart, then it will be time to get help for anxiety which can take over your life and ruin the good time you should be having at this age.

Sorry Correction here (Sometimes I also feel pain in the nail of center finger of right hand that does takes log time to go.) its in the left hand center finger nail.

Hi Thanks for your response. The pain start any time, whether I am sitting in the office, laydown or walking. but when I rest during night I do not feel such pain. It does not move from shoulder to hand. sometimes its in the shoulder, sometimes only arm and hand.
Now my I am getting more worried because, when I go home and laydown for sleep or watch tv, its not there till morning. when I awake and start getting ready for office it starts. Sometimes I also feel pain in the nail of center finger of right hand that does takes log time to go. Pain is mild in nature not that much severe.
I also feel lazy

I have consulted almost 3,4 doctors. 1 Cardiologist and 3 Internal medicine.  
can you pls guide after Normal angiography could It be due to heart problem or anxiety is killing me.?

Can you describe the pain you now have in your left arm and shoulder?

Does it appear when you move your arm in a certain way, or when you lift something?  When you are physically working hard, or perhaps only when you lie in a certain position in bed, as in on your side or on your back?
