Heart not working properly

A very close family member of mine have been having troubles whilst trying to sleep as he will be short of breath and a result of such he has to "sleep" whilst sitting. His breaths have become very deep lately and very noisy. When he went to hospital he was admitted for a week and the doctos said that he has a blood cloth for which he was given Warfarin and has been taking it every day for the past 2 months and that his heart is not beating properly anymore. All of a sudden his bottom part of the legs have developed lots of red spots and his feet are now a one whole red bruise, leaving him with a lot itching! The doctors said that they do not know what is the reason for these spots!!

What i am concerned the most is why is his heart is not beating properly and his constant short of breaths! He has had by-pass done before! What is the next step to take? do you think angiogram would be the answer?

I tried to tell him to go to emergency again but he would not listen, so i am trying to take this in my hands!

Your help would be greatly appreciated as i am scared i wont have this person long with me! :(

The red dots could be a sign of too much warfarin and are listed as a side effect of overdose. How long ago did your family member have a blood test to check the INR number? This is important to ensure the clotting factors are within the expected limits.
With regards to the heart. Are they suspecting a pulmonary embolism? which can of course be verified using a CT pulmonary angiography (CTPA).
A PE can put strain on the right side of the heart and increased pressure on the supply to the left side of the heart.Does your family member have swollen ankles?
