: My father, a 56 year old heavy smoker, lives in Romania and has a very
advanced form of ischemic cardiopathy. He had an angiogram done in Romania
earlier this year, and he was ruled out of any type of bypass surgery
because of the very advanced state of his disease. He takes medication
and has been under physicians' scrutiny, but the disease advanced,
and at this point he very frequently has severe chest pains. Very often,
the pains make it impossible for him to get his night sleep. I have been
reading recently about new drug tests, and in view of this, is there any
new medication or other type of treatment that might help him? I would
be very grateful if you could give me an advice.
  Thank you!

Dear Paula,
Ischemic cardiomyopathy is a difficult problem to treat with medications alone especially when percutaneous intervention with balloon or bypass surgery are not possible. The different groups of medications that are available include nitrates, aspirin, blood thinners, beta blockers, calcium channel blockers and ACE inhibitors. Cholesterol lowering medications also may help in the long run. In each of these class of medications there are many new medications available with subtle differences in their properties. It is extremely difficult to suggest you something specific without knowing details about your father
