I'm a bit worried

Hi, I'm an 18 year old male. I smoke cigarettes and drink coffee every day. Heart disease runs in my family. Other than those things, I have no known risk factors for heart disease.

Here's my question:

I have had an acute chest pain right around my heart off and on for several years now. I've been to the doctor before about it, and they attributed it to Costochondritis and/or Pleurisy, and told me it would go away. It never did, though. I would describe the pain as a sharp, stabbing one sometimes accompanied with a "gurgling" or "fluttering" sensation. It's usually right at the base of my sternum, somewhat on the left side. It's really wierd. And it usually only happens when I'm sitting down or walking...rarely ever during physical exertion. And I'm in great physical shape! It may happen several times throughout the day a few days a week, then it may disappear for a month or two. But it always comes back.
